The National Guild of Hypnotists recommends that all certified hypnotists offer a Client Bill of Rights. This is NOT a legal contract. It could more accurately be called "a memo of understanding." It outlines your rights as a client. You can download it, print it out, sign it, and bring it to our first session, OR we can go over it together in my office.  The text is also below. You keep a copy, and I keep a copy.

Contact Info: Jerome Masek sees clients for in-person sessions at a site in the Cleveland neighborhood of West Park. Zoom sessions originate from my home office, 19560 Puritas Ave. #132, Cleveland, OH 44135. Call me at 216-210-5504, send e-mail to JeromeMasekCH@aol.com, or visit www.JeromeMasek.net/hypnosis.

Education & Training: In 2005, I trained with certified instructors Patricia MacIssac, LPN., and the late Edward Hightower, M.S., through the National Guild of Hypnotists, a professional organization founded in 1950. I take continuing education credits annually to maintain my training. From 2005-2008, I was a member of Mind·Body Spirit Connected (MBSC), a local non-profit that provided education on complementary and alternative therapies and a holistic approach to health. In 2008, MBSC merged with the American Holistic Medical Association (AHMA), a national organization founded in 1978 to help transform healthcare to a more holistic model. I served on the Board of the Cleveland Chapter. In 2014, AHMA merged with the Academy of Integrative Health and Wellness (AIHW).

Ohio has not adopted education and training standards for hypnotism. This statement of credentials is for informational purposes only. Hypnotism is a self-regulating profession; practitioners are not licensed by state government. I am neither a physician nor a licensed health-care provider. I cannot provide a medical diagnosis nor recommend discontinuance of medically prescribed treatments. If a client desires a diagnosis or any other type of treatment from a different practitioner, the client may seek such services at any time. If my services are terminated by a client, the client has the right to coordinated transfer of services to another practitioner. A client has the right to refuse hypnotism services at any time; a right to be free of physical, verbal or sexual abuse; and a right to know the expected duration of sessions. A client may assert any right without retaliation.

Redress: As a Certified Member of the National Guild of Hypnotists, I practice in accordance with its Code of Ethics and Standards. If you have a complaint about my services or behavior that I cannot resolve for you personally, you may contact the NGH, PO Box 308, Merrimack, NH, 03054-0308, or call 603-429-9438, to seek redress. Services other than my own may be available to you. You can locate such providers on-line.

Fees: $75 per in-person session, $55 per Zoom session. Fees for consultations and group sessions are available. Pay by cash, check or Venmo before the session. Make checks payable to The Jerome Group LLC.

Confidentiality: I will not release information to anyone without written authorization from you, except as provided for by law. You have the right of access to my written record about you.

Insurance: I suggest that you think of my services as something that you will pay for personally. That will both protect your privacy and help you value the work you are doing more. In general, insurance companies are not likely to cover hypnotic services, and I caution you not to expect them to do so. In the future, this may change.

My Approach: I work for your highest and best purpose, for your success is my success. If you are ready to make changes to improve your quality of life, hypnosis can be a powerful, effective tool. I obtain a great deal of satisfaction from helping people, and I look forward to working with you to help you to achieve your goals.
